2012-13 Enrollment Information:

For new enrollments, please call the Welcome Center at 859-824-4184 to schedule an appointment so you can complete the paperwork necessary to get your child registered for school.

Are you New to Kentucky Schools?

Below is a list of documents you will need to enroll your child in school if they are being enrolled in a Kentucky school for the first time. 

  • Legal Birth Certificate
  • Kentucky Certificate of Immunization
  • Proof of a Preventative Health Care Examination, documented on the appropriate Kentucky form
  • Proof of an eye examination by an optometrist or ophthalmologist, documented on the appropriate Kentucky form
  • Proof of a Dental Screening or examination (for 5 or 6 year olds), documented on the appropriate Kentucky form

 You will also need:

  • Proof of Custody/Guardianship (if applicable)
  • 2 Proofs of Residency (ex: rental/lease agreement, purchase contract, utility bill)
  • Previous School Information (Name, Address, Phone and Fax Number)

 

 

Are you transferring from a Kentucky school?

You will need the following documents to transfer your child from a Kentucky school to a school in our district.

  • Proof of Custody /Guardianship (if applicable)
  • 2 Proofs of Residency (ex: rental/lease agreement, purchase contract, utility bill)
  • Previous School Information (Name, Address, Phone and Fax Number)

All Out of District Students applying for enrollment in our school district will need to complete and submit an Out of District Application & Request for Records for Consideration as an Out of District Student before beginning the enrollment process.  Applications & Request forms may be picked up from the Welcome Center or you may be print out the forms from the forms tab above and then return them to the Welcome Center upon completion.
